Effective dosage is 1ml to 2ml per 1ltr of fuel; MilEx (diesel engines) or MPH (petrol engines). For example a 60ltr tank will use 120ml fuel additives, which is equivalent to one pouch.
Recommended is to use one pouch per full tank, however, the additives benefits will still be noticed if you use a pouch every other full or half tank.
